A service saying “insurance may help” is not the same as a plan confirming a covered claim. Record who said what.

Is a pharmacy-made sermorelin claim covered under my plan?

What product name and billing code will the pharmacy put on the insurance claim? (Ask them to read it out so you can give it to the plan.)

Does the insurance company need your records, or its own permission before treatment starts (called prior approval)?

Are the ordered labs covered, and where can they be done?

Are the clinician, lab and pharmacy each in network?

Keep the plan’s answer with the date, the name of the person who answered and any reference number they gave. That makes a later question much easier.
